A new payment option starting in 2025. This payment option helps you spread out the cost of your medicine across the calendar year. There is no cost to participate in the Medicare Prescription Payment Plan.

With a Community Health Choice (HMO D-SNP) plan, preventive health care is covered at no cost to you. With this knowledge you and your doctor can work together to develop an action plan and identify adjustments to risk factors you can control.

You’re eligible if you qualify for Medicare Part A and B, live in our service area and qualify for Medicaid QMB, QMB+, or SLMB+. When you are ready to enroll, you may either enroll online or print an application and enroll by mail.
