STAR+PLUS Launching on September 1st!

We have some exciting news to share with you! On September 1, 2024, Community Health Choice will start offering STAR+PLUS, a Medicaid-managed care program.

STAR+PLUS helps Texans with disabilities and those who are 65 and older get the healthcare they need to live a healthy life. Let’s take a look at what STAR+PLUS is all about and how it can help you or your loved ones.

What is STAR+PLUS?

STAR+PLUS is a Texas Medicaid program that serves older and disabled adults through the healthcare plan they choose. This program has many ways that it helps improve the quality of life for members. Here are some things that it can help with:

  • Complete Care: Includes medical, behavioral health, and long-term services and supports.
  • Help with Daily Life: Helps with things like bathing, dressing, and cooking meals.
  • Service Coordination: Each Member gets a Service Coordinator to help make their own personal care plan.
  • Care at Home: Helps Members get care and help to be able to stay in their own home.

Who can get STAR+PLUS?

To get services through STAR+PLUS you must: (1) be approved for Medicaid, (2) be one or more of the following:

  • Age 21 or older, getting Supplemental Security Income (SSI) benefits and able to get Medicaid due to low income
  • Not getting SSI and need the type of services in STAR+PLUS Home and Community-Based Services
  • Age 21 or older, getting Medicaid through what are called “Social Security Exclusion programs” and meet program rules for income and asset levels
  • Age 21 or over residing in a nursing home and receiving Medicaid while in the nursing home
  • In the Medicaid for Breast and Cervical Cancer program

How to Enroll for STAR+PLUS

Enrolling in STAR+PLUS is easy. There are three ways to apply. Here’s how you can get started:

Apply online

You can fill out the form at your own pace. Visit www.yourtexasbenefits.com. If you have questions, call us for help toll-free at 1.888.760.2600.

Apply by phone

Call 2-1-1. When you call, pick a language, and then, press 2. A Texas Health and Human Services Commission (HHSC) representative will start your application over the phone, then mail it to you for you to sign. Send your application back to HHSC with the items they need to prove your income and receipts showing what you pay for things like childcare, alimony or care for an adult who has a disability. You can use the pre-paid envelope or fax everything to 1.877.447.2839. It can take up to 45 days for HHSC to work on your application. You can call 2-1-1 for updates.

Apply by mail or fax

You can print an application by clicking here. Send your application to HHSC with the items they need to prove your income (paycheck stubs) and receipts for things like childcare, child support or care for an adult who has a disability. You can use the pre-paid envelope or fax everything to 1.877.447.2839. It can take up to 45 days for HHSC to work on your application.
